ABSTRACT

Malignant peripheral nerve sheath tumors (MPNSTs) are rare and aggressive soft tissue sarcomas of neural origin. Approximately 50% of MPNSTs are associated with neurofibromatosis type 1 (NF1), a neurocutaneous condition characterized by skin discoloration. MPNSTs occur in approximately 4% of patients with NF1. Malignancy should be suspected when a large mass consistent with a neurofibroma is observed with heterogeneity on a radiologic examination. In our case, immunohistochemistry revealed the presence of antigens for both the tumor protein p53 and the proliferation marker Ki-67 (MKI67). In particular, MKI67 positivity helped to differentiate MPNST from neurofibroma. Complete surgical resection is the standard treatment. After surgery, radiation therapy is typically administered to the resection area to reduce the likelihood of recurrence. Following treatment, patients should undergo regular clinical follow-up using a combination of magnetic resonance imaging, computed tomography, and bone scanning for several years to monitor them for possible metastasis.

ABSTRACT

Background@#Soft tissue fillers are increasingly used to rejuvenate and beautify the body and face. Unknown foreign materials that have not always been proven to be safe for human use have been used for injections in the past. Various commercial injectable materials have since been developed and are used; however, the number of complications has increased as soft tissue fillers have increased in use. @*Methods@#A retrospective study of 39 patients with facial foreign body complications who underwent surgical treatment between March 2012 and February 2021 was conducted. Patient information was analyzed, and patient satisfaction was evaluated using a questionnaire after surgical treatment. A comparative statistical analysis was undertaken. @*Results@#Patients who underwent procedures performed by unlicensed practitioners tended to be older at the time of presenting to the hospital (P=0.004). The degree of satisfaction after surgical repair was significantly lower following the direct approach than after the indirect approach using a bypass incision (P=0.044). The degree of satisfaction significantly decreased (P=0.001) as the number of operations the patient received increased. @*Conclusions@#When treating complications caused by foreign material injections, surgical removal is a more reliable method than treatment with medication or further injections; however, treatment decisions should be made thoughtfully, and many factors must be considered. Patient satisfaction can be improved by reducing the number of operations and making the appropriate corresponding incisions or excisions to treat the full range of symptoms caused by foreign substances and by indirectly using bypass incisions to mitigate post-surgery scars.

Background@#Herpes simplex virus (HSV) infection is a common viral disease of the skin worldwide. HSV type 1 (HSV-1) has been associated with orofacial infections, while HSV type 2 (HSV-2) accounts for the main cause of genital herpes. However, a number of recent clinical studies have revealed that the epidemiology of HSV-1 and HSV-2 is changing. @*Objective@#We performed the study to find out if there is any correlation between HSV subtypes and other multiple variables. @*Methods@#We examined the results of HSV-polymerase chain reaction (PCR) tests performed in our hospital from July 2017 through June 2020. We analyzed the HSV subtypes of samples from 304 patients that showed positive HSV-PCR. Comparative analysis was performed to identify the correlation between HSV subtypes and other multiple variables. @*Results@#HSV-1 was typed in 53.9% (164/304) and HSV-2 in 46.1% (140/304) of the patients during the study period. HSV-1 showed predilection for body area above the waist, whereas HSV-2 was dominant below the waist. There was a statistically significant correlation between the HSV subtypes and site of infection (p<0.001). Female patients were more likely to acquire HSV-2 infection than male patients (p=0.027). The ratio of HSV-2 infection tended to get bigger as the age of patients increased (p<0.001). There was not a significant difference in HSV subtypes among the groups defined according to immune status (p=0.333). @*Conclusion@#Our study provided abundant data concerning various aspects of epidemiology of HSV infection. They can be used to plan and evaluate strategies for management of patients.

Background@#Isotretinoin is a gold standard treatment for moderate to severe acne vulgaris but is associated with cheilitis. Though moisturizing agents or petrolatum have been suggested, uncomfortable isotretinoin-induced cheilitis can disrupt treatment of acne patients. Topical tacrolimus has been used in atopic dermatitis with good safety and efficacy; however, there is no study of application of topical tacrolimus in cheilitis induced by isotretinoin. @*Objective@#In this study, we aimed to describe the efficacy of topical tacrolimus 0.1% ointment on cheilitis associated with isotretinoin therapy using isotretinoin cheilitis grading scale (ICGS), investigator global assessment (IGA), and patient global assessment (PGA). We also observed the side effects of topical tacrolimus ointment. @*Methods@#Fifty acne vulgaris patients with isotretinoin-induced cheilitis were randomly allocated to either topical tacrolimus or petrolatum treatment using permuted-block randomization. Patients were followed-up at 4 and 8 weeks, at which cheilitis lesions were photographed and evaluated with ICGS, IGA, and PGA. @*Results@#Compared to petrolatum group, tacrolimus group had greater responses to treatment as measured by mean values of ICGS, IGA, PGA at follow-up visits. Also, the ICGS decrease was larger in the tacrolimus group compared with the petrolatum group even according to isotretinoin dose. @*Conclusion@#Topical tacrolimus ointment had superior efficacy in treating cheilitis induced by isotretinoin compared to petrolatum. Erythema, fissures, scales, and commissures all showed better response to tacrolimus ointment. Topical tacrolimus ointment can be administered as an effective strategy in treatment of cheilitis as a complication of isotretinoin therapy and can improve compliance of acne patients.

ABSTRACT

Viral warts are benign proliferations of the epithelium caused by human papilloma virus (HPV) infection. Diverse therapeutic options are available for viral warts, depending on extension and severity of the disease. We report a case of a 19-year-old man who presented with multiple viral warts on hands and feet for 5 years. He was treated at other clinics before visiting our hospital, but there was no improvement. We treated the lesions with a combination therapy of systemic acitretin and diphenylcyclopropenone (DPCP) immunotherapy for 6 months. A significant improvement was observed during the 12th week of therapy. Herein, we report a case of recalcitrant viral warts showing complete regression when a combination therapy of oral acitretin and immunotherapy was administered.

ABSTRACT

We report a 29-year-old female with a one-month history of non-healing multiple erythematous to violaceous plaques with crusts over both legs and feet. Tender, scarring ulcers with surrounding erythema were present. The clinical manifestation, together with histopathologic findings of fibrinoid plugs within vascular lumens and walls, as well as red blood cell extravasation, led to diagnosis of livedoid vasculopathy.The patient experienced recurrent painful violaceous plaques with ulcerations during the two years of treatment with oral pentoxifylline 400 mg three times daily. The cutaneous lesions and symptoms dramatically improved after the treatment regimen changed to oral sulodexide (250 lipasemic units) three times daily. Sulodexide, a highly purified mixture of glycosaminoglycans including dermatan sulfate and lowmolecular weight heparin, could be an effective therapy for recalcitrant livedoid vasculopathy. Herein, we report a case of livedoid vasculopathy treated with sulodexide, which has not previously been reported.

ABSTRACT

While lymphangioma circumscriptum may present anywhere on the body, the vulva is a rare site. However, vulvar involvement can occur in some clinical settings, and Crohn’s disease is one of the underlying etiologies of acquired lymphangioma circumscriptum. A 31-year-old woman presented with an 8-year history of widespread verruciform plaques on her vulva. She complained of a mild itching sensation at the lesion site. She was diagnosed with Crohn’s disease 11 years prior but had not been treated. She had no history of trauma or surgery in the perineal area and no familial history of cancer or hereditary disorders. The histopathologic findings were consistent with lymphangioma. Based on the clinicopathologic findings, the patient was diagnosed with lymphangioma circumscriptum on the vulva. Herein, we report a rare case of acquired lymphangioma circumscriptum of the vulva mimicking condyloma acuminatum in a patient with Crohn’s disease.

ABSTRACT

BACKGROUND: Motorcycles are one of the important members of commercial transportation because of the convenient use during congested traffic conditions and the ease of parking in narrow streets. This study investigates the characteristics of crashes and traffic signal violations caused by motorcycle couriers. METHODS: From the national compensation data, this study analyzed the traffic crashes caused by 671 motorcycle couriers. RESULTS: Among 671 injured couriers, 50.6% were aged less than 40 years, 49.2% run in a small business of < 5 employees, and 47.2% had work experience of < 6 months. Motorcycle crashes occurred mainly due to “rider overturned alone” (67.5%), in the daytime (73.5%), or on cloudy or clear days (77.2%). However, the violation rate caused by motorcycle couriers was high in couriers in a small business of < 5 employees (13.9%), with work experience of < 6 months (13.9%), on cloudy or clear days (12.4%), on an intersection (29.8%), in the type of “crash with a vehicle” (31.2%), or in a death accident (35.7%). CONCLUSION: The findings of this study can be used as a baseline in devising policies for preventing crashes of motorcycle couriers.

ABSTRACT

Neuroendocrine carcinomas mostly occur in the gastrointestinal tract, lung and pancreas. The gallbladder is an uncommon organ for a neuroendocrine carcinoma and it is difficult to diagnose before gallbladder surgery. Owing to its rare occurrence its natural course and treatment are not definitely established. A 71-year-old woman was hospitalized with intermittent abdominal pain and nausea. Abdominal computed tomography showed multiple masses of the gallbladder obstructing the biliary tract. From endoscopic ultrasonography we suspected carcinoma of the gallbladder that was infiltrating a common bile duct. A biopsy was taken using endoscopic retrograde cholangiopancreatography (ERCP) and the mass was diagnosed as a neuroendocrine carcinoma of the gallbladder. Here we report a case of a neuroendocrine carcinoma of the gallbladder confirmed by endoscopic biopsy, and provide a review of the literature.

ABSTRACT

PURPOSE: The purpose of this study was to observe and quantify the expression of interleukin-4 (IL-4), interferon-gamma (IFN-gamma), and tissue inhibitor of matrix metalloproteinase-2 (TIMP-2) in the gingival tissue of patients with type 2 diabetes mellitus (DM) and healthy adults with chronic periodontitis. METHODS: Twelve patients with type 2 DM and chronic periodontitis (Group 3), twelve patients with chronic periodontitis (Group 2), and twelve healthy individuals (Group 1) were included in the study. Clinical criteria of gingival (sulcus bleeding index value, probing depths) and radiographic evidences of bone resorption were divided into three groups. The concentrations of cytokines were determined by a western blot analysis and compared using one-way ANOVA followed by Tukey's test. RESULTS: The expression levels of IFN-gamma and TIMP-2 showed an increasing tendency in Groups 2 and 3 when compared to Group 1. On the other hand, the expression of IL-4 was highest in Group 1. CONCLUSIONS: The findings suggest that IFN-gamma and TIMP-2 may be involved in the periodontal inflammation associated with type 2 DM. IL-4 may be involved in the retrogression of the periodontal inflammation associated with type 2 DM.

ABSTRACT

PURPOSE: The purpose of this study is to evaluate the long term clinical and radiographic outcome and stability after transplantation of third molar with complete root formation. METHODS: The subjects were 31 teeth (male 17, female 14, aged 22-55, average 39.9 yr old) of 31 patients who visited the department of periodontics and passed more than two years after autotransplantation procedure and still under regular check up. Modified success criteria of Chamberlin and Goerig was applied to determine the success of autotransplantation. RESULTS: Three out of 31 teeth failed and resulted 90.3% of success rate. When compared according to sex, 15 out of 17 teeth had succeeded in male, 13 out of 14 succeeded in female. When compared the success rate according to cause of extraction, tooth loss due to caries and root fracture had all succeeded but 3 out of 24 had failed in tooth loss due to periodontal disease. When compared according to donor teeth, 12 out of 14 maxillary third molars and 16 out of 17 mandibular third molars had succeeded. CONCLUSIONS: In long term evaluation over two years, if appropriate surgical procedure and proper case selection is made, autotransplantation of the third molar with complete root formation can be the alternative choice that substitutes prosthetic or implant treatment and it is a functionally acceptable procedure.
